AGED 0-5 YEARS – NO COST
PROVIDED THAT ONE PARENT IS REGISTERED ON OUR PLANS
SEEN ACCORDING TO NEED AND RISK LEVEL: Every 6 months or every 12 months for a check up.
PLAN D – 6-12 YEARS – £5.95 per month
2 Dental Examinations per year
Including tailored oral hygiene instruction, dietary advice and fluoride varnish application
PLAN E – 13-18 YEARS – £11.50 per month

How do these appointments help your child?
Dental health examinations provide:
• Thorough examination of their current dental health to catch developing problems early
• Checking for signs of oral cancer for early detection
• X-rays, where clinically necessary, for an internal examination of their oral health.
Hygiene appointments provide:
• Professional removal of harmful hard plaque
• Advice and techniques – we work with them to prevent tooth decay and gum disease

How Do You Join Our Dental Plans?
There is no need for an assessment. Joining is very simple.
All you have to do is complete a registration form, a Direct Debit mandate and an authorisation form for Practice Plan. In addition to your first monthly payment, an initial registration fee of £10 will be payable to Practice Plan and will be included in your first Direct Debit. If you choose to leave the plan for any reason you can do so by simply giving us one month’s notice.
